Healthcare Workers (Nurses and Carers) Registration
- 2 passport-sized photographs.
- Completed application form.
- Proof of identity ( Passport /Birth Certificate/Driving licence. British citizenship, certificate, indefinite leave to remain, etc).
- Proof of address
- National Insurance Number (NI)
- Professional Qualifications /Certificates.
- Work Experience.
- Minimum of One year experience for Carers/support workers
- References (Name, Addresses, email, and Telephone Number of 2 people who can provide you with references. (This must be a charge nurse/sister/senior level staff).
- DBS Disclosure
- Bank details.
- P45 or equivalent.
- Proof of professional registration. NMC PIN NO. (nurses)
